Ballet BC 44 Summer Intensive/ July 22 - August 3, 2024/ 1286 Cartwright St, Vancouver BC

1) I recognize that ballet and contemporary dance may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

2) I understand that it is my responsibility to consult a physician regarding any and all matters involving my personal health and including without limitation: (a) any prior or existing injuries; and (b) my ability to participate in the summer intensive offered by Ballet BC.
3) I freely and voluntarily accept and fully assume all risks, hazards, dangers and liabilities inherent in the use of Ballet BC’s studios and participation in the summer intensive.

4) I agree to observe any health and safety policies that Ballet BC may implement to reduce the risk of Disease and other physical harm or injury.

5) I hereby waive any and all claims I may have against the Pacific Ballet British Columbia Society (dba Ballet BC), its staff, artists and instructors, and release them from all liability. I agree not to sue for any personal injury, death or damage to the person or property or loss sustained as a result of my participation in Ballet BC 44 Summer Intensive.

6) Ballet BC is not liable for damage and/or theft of personal belongings.

7) I understand that I must arrange for health coverage for the duration of my time in Vancouver.

I hereby grant Pacific Ballet British Columbia (dba Ballet BC), its representatives and employees, permission to take photographs and video of me and my property in connection with Ballet BC 44 Summer Intensive. I authorize Ballet BC, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Ballet BC may use such photographs or video of me with or without my name and for any lawful purpose, including such purposes as publicity, illustration, advertising, social media and web content. I understand that I will not own the rights regarding these materials.
